The Health-Related Social Needs (HRSN) program helps eligible Oregon Health Plan (OHP) members stay safely and stably housed by providing short-term rent and housing support when a person’s health is at risk due to housing instability.

HRSN Rental Assistance Eligibility & Application

Through our partnership with a certified Trillium HRSN provider, ORHF can help eligible individuals access Medicaid-funded rental assistance to stabilize their housing during recovery. This benefit can help cover rent payments for people who qualify under Trillium’s Health-Related Social Needs (HRSN) program.
To qualify, you must:

Be enrolled with Trillium Community Health Plan or have Oregon Open Card (fee-for-service Medicaid)

Have an active lease or rental agreement

Meet HRSN criteria for housing instability
Documents required:

Your current lease

A signed attestation form (confirming your living situation and need)

W-2 or income documentation, if applicable

Any other supporting documents relevant to your situation
